Patient Rights & Responsibilities

Last updated: November 2026

Sandipan Memorial Health Care is committed to providing ethical, transparent, and patient-centred care. This charter outlines the rights of every patient who visits our facility, and the responsibilities we ask of them in return, to ensure a safe and respectful environment for all.

Part 1

Your Rights as a Patient

1

Right to Respectful Care

Every patient is entitled to receive courteous, respectful, and dignified care at all times, regardless of age, gender, religion, caste, economic status, or disability.

2

Right to Informed Consent

Before any diagnostic procedure or clinical intervention is performed, patients have the right to receive a clear explanation of what the procedure involves, its purpose, any associated risks, and any alternatives available. Procedures requiring written consent will be carried out only after such consent is obtained.

3

Right to Privacy and Confidentiality

All patient information, including medical history, test results, and personal data, is kept strictly confidential. Information is shared only with the patient, their authorised representative, or the treating doctor, except where required by law.

4

Right to Access Medical Records

Patients have the right to access their own diagnostic reports and medical records held by Sandipan Memorial Health Care. Requests may be made to our reception, and records will be provided in a reasonable time.

5

Right to Know the Cost of Services

Patients are entitled to know the cost of every service before it is rendered. All charges are disclosed upfront, and itemized printed receipts or digital invoices are provided upon completion of service. There are no hidden charges.

6

Right to Refuse a Service

Patients have the right to refuse any test, procedure, or service after being informed about the consequences of such refusal. Refusal will be documented where clinically relevant.

7

Right to a Second Opinion

Patients have the right to seek a second clinical opinion from another registered medical practitioner at any time. We will provide copies of reports and relevant documentation to facilitate this.

8

Right to Non-Discrimination

Services at Sandipan Memorial Health Care are provided without discrimination on the basis of religion, caste, language, gender, sexual orientation, disability, or economic status.

9

Right to Safe and Hygienic Care

Patients have the right to receive care in a clean, safe, and hygienic environment. All diagnostic equipment and collection materials are sterile and maintained to clinical standards.

Part 2

Your Responsibilities as a Patient

1

Provide Accurate Information

Patients are responsible for providing complete and accurate information about their health history, current medications, allergies, and any previous diagnoses. Incomplete information may affect the quality of your diagnostic results.

2

Follow Pre-Test Instructions

For certain tests (such as fasting blood glucose or lipid profiles), specific preparation may be required. Patients are responsible for following the instructions provided at the time of booking to ensure accurate results.

3

Arrive on Time

Patients with appointments are requested to arrive punctually. Late arrivals may result in rescheduling depending on availability.

4

Treat Staff Respectfully

All patients and accompanying visitors are expected to treat our medical and administrative staff with respect and courtesy. Verbal abuse, harassment, or threatening behaviour will not be tolerated and may result in refusal of service.

5

Respect Other Patients

Patients are expected to maintain the privacy and comfort of other patients within the facility. Noise, disruption, and behaviour that affects other patients is not acceptable.

6

Settle Bills Promptly

Patients are responsible for settling all applicable charges for services received. Disputes regarding billing should be raised with reception at the time of service.

7

Collect Your Reports

Patients are responsible for collecting their reports within a reasonable time after they become available. Digital delivery is provided as a convenience, but the physical or official digital report must be taken by the patient.

8

Follow Clinical Guidance

Patients are encouraged to follow the advice of their treating doctor regarding the interpretation and follow-up of any diagnostic findings. Our reports are clinical documents intended for use by a qualified medical practitioner.
